Answer the following questions as honestly as possible. Your answers will help assess whether counseling could benefit you and what type of counseling might be most helpful.

Zone A
I often feel stuck in patterns or situations I can’t seem to change on my own.
I struggle to manage my emotions, such as anger, sadness, or worry, in a healthy way.
I feel overwhelmed or unsure about how to handle challenges in my life.
I find it difficult to maintain healthy relationships or communicate my needs effectively.
I wish I had more support or guidance in navigating my thoughts, feelings, or life decisions.
Zone B
I often feel emotionally overwhelmed by memories or events from my past.
I avoid certain people, places, or situations because they remind me of something painful.
I experience nightmares or flashbacks related to past events.
I feel on edge or overly alert, even in situations where I’m safe.

Scoring Key:
Add up your scores for each section (out of 15).
0–4: You may not need counseling in this area.
5–10: You could benefit from counseling, especially if these challenges are persistent or impacting your daily life.
11–15: Counseling may be particularly helpful in addressing these issues.

These questions are intended to help a person understand more about their general need for counseling. The overall score for this section is not as important. The goal is to explore one’s overall quality of life and to determine if the counseling would be sufficiently beneficial.
